STUDENT RECORDS & TRANSCRIPTS  

The Records Office is open daily Monday - Friday 7:30 a.m. to 4:00 p.m.

High School Transcript Request:

​

  • Transcripts can be requested by completing a request form.

  • Your transcripts will be processed within 48 business hours after submitting the form. 

  • Official transcripts are $3.00 per copy (Cash, Cashier Check, or Money Orders payable to SMHS will be accepted) There is no fee for unofficial transcripts.

  • Graduation verification from third parties (such as background checks) is also $3.00.

  • All requests need a photo ID

  • Fax: A written request may be faxed by the student to the registrar at (805) 314-2739, or emailed to smhs-records@smjuhsd.org

  • Please include the following -                                                                                                                                                                                                                         1) Full name of the student (name used while in High School)

                     2) Copy of picture ID

                     3) Complete address where the transcript is to be mailed

                     4) Graduation year

  • Mail: Request can be mailed to 901 S Broadway, Santa Maria, Ca 93454 with request form, Photo ID, and $3.00 -  Attention Registrar Eileen Gomez

  • Seniors: at the end of the school year, submit a request to have your transcript sent to your forwarding College/University
